Kyoto hole
Here we go, photos of the infamous hole. I tugged on a weirdly thick row of a stitches, I think that I might have doubled the yarn over when adding a new skien for more than my usual two stitches.

It looks like a magical increase to me. Perhaps you can use the bit on the back to fudge it in duplicate stitch?
BTW, your haircut is super-cute! Cuter than the logo graphic's (which is already pretty darn cute)

Something similar happened to me while knitting something or other...luckily I caught it and could fix it...but it was a case where I started knitting at the wrong side and short-rowed, then picked it up later on the correct side and knitted a few more rows. Then I saw the hole and when I frogged back, I noticed the cause of the problem.

Though I am sure you are frustrated beyond return with Kyoto, maybe this one is worth taking to a yarn store for help? Just looking at it I'm not sure what went wrong, but the middle picture makes it look like a dropped stitch. Hmm...good luck!
